We want to provide the best quality health care we can. Please help us help you more efficiently.
When leaving a message, please leave the spelling of your first and last name, your date or birth, and a phone number where you can be reached. Your date of birth is really important to locate the proper chart. We have patients with similar names, and we want to make sure we have the right chart! Please do state the reason for your call so the provider can be ready to help you. You can also use our Nurse Question Form for non urgent medical questons. Please call again if the condition changes, or if you have not had a call back within 24 hours. If this is an urgent message during office hours, please press 2 to speak to our staff. Please do not leave urgent messages in the nurse/provider call back mailbox as this mailbox only gets checked once/hour when the office is open. After office hours, please leave a message in the urgent mailbox if it is for a medical problem that cannot wait until the office reopens. Please make sure you leave the spelling of your first and last name, date of birth and a phone number where the provider can contact you. If it is a life threatening problem, please dial 911.
If you are calling in for a prescription refill, please leave us a pharmacy number, or let us know if you will be picking up the prescription. We also need your spelling of your last name as well as your birth date, the name of the medication, strength and how often you take it. You can request prescriptions online.
If you are having a physical exam, you can download the appropriate forms, either the Medical History Questionaire for adults, pediatric and adolescent, or school physical from this site. You will also need to fast from midnight on the night before the visit so the lab work can be completed. You may have water, or black coffee or tea with no sugar or cream after midnight.
Lab results may take up to 5 days to come back. Pap smears take a few weeks. Please let us know if you have not received your results after this time frame.
